HEWT Short-Term and Long-Term Disability Plans

HEWT participants are eligible for both Short-Term and Long-Term benefits. If you choose to enroll, your employer will pay for 50% of the Short-Term Disability monthly premiums. All HEWT participants are eligible for Long-Term Disability benefits which are provided at no cost to you. These benefits provide you with income replacement while off work due to a non-work related illness or injury. The first 35 days of disability are paid through your employer’s payroll system after the appropriate waiting period. Thereafter, the remaining period of approved disability is paid by New York Life. Both Short and Long-Term Disability are an employee-only benefit.

Short-Term Disability

Short-Term Disability (STD) benefits replace a portion of your income when you are unable to work due to a non-work-related illness or injury. The opportunity to enroll in Short-Term Disability is at the time of hire, or during annual enrollment. If you enroll, your employer will pay 50% of the monthly premium and you will be responsible for the remaining 50% of the premium. Payroll deductions will be on an after-tax basis and only 50% of the weekly paid benefit will be taxable.

How do I file a disability claim?
  • Print a Physician's Certification and Disclosure Authorization Form (Site Form A-6003-497) or click link.
  • The employee must complete and sign Part 1 and Part 3 of the form.
  • The physician or health care provider must complete and sign Part 2 of the form.
  • Once the form has been completed in its entirety (Parts 1, 2 and 3) it should be mailed or faxed to the following address or telephone number.

Claims must be filled within 31 days from the onset of the disability. Once your claim is submitted, you will begin receiving disability payments, up to the first 35 days, through the normal payroll process while your claim is being reviewed for eligibility. If your claim is subsequently denied, any benefits paid to you for the disability must be returned to your employer. Any STD benefits paid after day 35 will be paid by the Third-Party Administrator, New York Life.


Long-Term Disability - Plan #NHD-960006

If you remain disabled for 180 consecutive calendar days due to any one disability, injury, or illness, you will be eligible to apply for Long-Term Disability benefits. Your employer will pay for a monthly benefit of 50% of basic earnings. The maximum monthly benefit is $4,000 decreased by all applicable reductions. There is currently no employee cost for this coverage.
